4 DNA Testing - Immediate Test Instructions

Read the entire instruction sheet before beginning. Only collect one person's sample at a time.

Note: In order to collect a proper sample it is important that each person has a clean mouth.
Adults and children: rinse mouth 3 times with warm water.
Infants: Allow infant to drink room temperature water from a bottle before sampling.
Do not use mouthwash or toothpaste.
Do not smoke prior to the testing.
Samples must be received by 4 DNA Testing no more that 3 days from sampling.

PREPARE TO SAMPLE

Gather the following items prior to sampling. Do not touch swab tips with your fingers.

• 2 sterile cotton-tipped swabs (cotton buds) for each test participant (can be purchased at most chemist's). If there are cotton buds on each end, cut off one leaving one end.
• 1 paper envelope per person tested. DO NOT USE PLASTIC BAGS OR SEALED TUBES. Paper envelopes allow samples to stay dry.
• 1 mailing envelope, size C5 (162x229 or 6x9 inches) or larger.
• Warm water for adults and children, room-temperature water in a baby bottle for infants.

OBTAIN SAMPLES

Step-by-Step

Rinse mouth 3 times with water (infants drink water).
2. Label the paper envelopes for each test participant with individual's name, race (please use 1 of these Caucasian (white), African, Asian, Hispanic or name other) and relationship with group.
   • Envelope 1............Name, Race, Relationship
   • Envelope 2............Name, Race, Relationship
   • Envelope 3............Name, Race, Relationship
   • Large Envelope.....To send all samples

When samples are complete, they will be put in the small envelopes and all of them will be put in the large envelope. The large envelope needs to be stamped and addressed to :

You are now ready to do the sampling.
3. DO NOT TOUCH THE TIP OF THE COTTON SWAB (BUD).
4. Collect sample by pressing and rolling the cotton swab firmly on the inside of each cheek 30 times (about 1 minute). Be certain to sample from the entire inner cheek surfaces as well as around the gums of the teeth, under the tongue and behind the lips.
5. Allow the swab to dry for 3 hours. Place swab in the envelope labelled with the person's name.
6. Repeat steps 3 and 4 with the second swab.
7. Seal the envelope - the first person is finished! Sample the next person and so on. (Start from Step 3 for each additional person)
8. Complete the Test Request Form and Tested Party Information.
9. Place in the large envelope:
   • Small envelope no.1 (Name, Race and Relationship)
   • Small envelope no.2 (Name, Race and Relationship)
   • Small envelope no.3 (Name, Race and Relationship)
   • Request form
   • Payment
10. Post immediately - it will take between 2-3 weeks after we have received the samples for you to obtain the results. If you have not received the results by then, please get in contact with us, using the Check Status form.
